Appointments

Patients are seen by appointments only. Please call the Texas Spine & Neurosurgery Center at (281) 313-0031 to schedule an appointment or fill out our contact form. Appointments are normally made by physician referrals, however, self-referrals are accepted in cases where the patient is seeking a second opinion or has known neurosurgical problems.

What to bring to your appointment?

  • Insurance card
  • MRI or CT (The actual films AND the paper report. CDs will not be accepted)
  • List of your current medications
  • Referring doctor’s address, phone, fax number
  • Referral, if required by your insurance

*When printing forms, please do not print front-to-back. Instead, limit one page per sheet of paper.

Insurance

We accept most commercial insurances, Medicare, Texas and Federal worker’s compensation. You will be required to have your insurance card and co-pay available at the time of your visit. We will assist you with the billing of your primary insurance. Letters of protection are not accepted.

Authorizations

It is your responsibility to ensure whether a service is covered on your insurance plan. You may need to contact your insurance company before making the appointment to determine this.

Confidentiality

We keep all patients’ medical records completely confidential. The records can only be released by your written authorization. We cannot discuss your medical condition and care with anyone, including members of your family without your permission.

Prescriptions

Prescription refills are available Monday through Friday 8:00 am – 4:00 pm. It is a good idea to order two days before you run out. Our neurosurgeons do not refill prescriptions on weekends or during evening hours.
